Your Application. This is a continuing application
and all changes to the information provided in your application
must be reported, in writing, (within thirty days of occurrence),
dated and signed. Refer to the applicable question number
for which the change applies and provide all details relative
to that question. You must continue to update your application
until you are admitted to practice law in Colorado. Be
certain to make copies of all changes prior to submitting
them to this Board.
Application Amendment Form. Use this form
to report changes to information in the application. (Keep
the original as a master from which to make copies for
reporting future changes as they occur.) This form may
not be used to register for a future exam.
Forms and Informational Materials(Section IV)
The supplemental forms contained
in Section IV are not subject to timely filing requirements.
Forms identified with the extension .rtf are designed
to be completed using your word processor and may be
saved to your system hard drive. Forms identified with
the extension PDF are to be printed in hard copy and
completed in pen and ink. Print and retain the informational
materials should be printed (or saved to your system
hard drive) for future reference. Once your application
is electronically submitted, informational materials
from Section IV are no longer accessible.

page and complete the information in pen. Your application
will not be deemed timely filed should you fail to submit
the Cover Page by the applicable filing deadline.
a. This form identifies the appropriate routing of your application,
thus ensuring fewer processing delays. It is also the means by which
your seating preferences for the examination will be determined.
b. Print your name legibly in the space provided.
c. Indicate on the cover page the examination month and year for which
you are applying; indicate right or left hand seating and whether you
will write or complete on lap top computer, your Essay/PT answers.
d. Indicate if you are requesting test accommodations for a temporary
or permanent disability, which will require non-standard testing or medically
prescribed equipment or devices during the examination and refer to the
section entitled Petition for Test Accommodations of these instructions
e. Indicate whether you have taken the Multistate Professional Responsibility
Examination (MPRE), the date of the exam, and your scaled score. You
must arrange to have an official notice of your MPRE scores sent directly
to this Board from ACT in order to meet the MPRE requirement. If you
have not yet taken the MPRE you should do so at the next scheduled administration.
Indicate the date on which you anticipate taking this exam.
f. Indicate whether the law school from which you received your first
professional law degree (JD or LLB) was accredited by the American Bar
Association. If your school was not ABA accredited at the time of your
graduation, you are subject to additional requirements. Please refer
to Rule 201.5(2)(a)(ii) or (iii), C.R.C.P. from Section IV for additional

a. Submit the completed accommodation forms no
later than the last filing deadline of the examination
for which you are making such request. Written
requests for testing accommodations should be submitted
no later than DECEMBER 1 for the February examination,
or MAY 1 for the July examination to allow sufficient
time for the Board to review your request.
b. Current supporting medical documentation must
accompany all requests. Requests submitted after
the official filing deadline will be deemed late
application and will be assessed late filing penalty
fees as set forth in the Information Page entitled
Filing Deadlines and Fees under Late Filing Penalty
Fees. In no case will requests submitted after
December 31 for the February examination or May
31 for the July examination be granted. Petitions
for accommodations filed late in the filing period
must be accompanied by all supporting medical documentation.
Incomplete petitions submitted after the last filing
deadline will be denied.
Ethnic Survey. Print
a hard copy of this form as a single page and complete the
information in pen. This document is used to track bar examination
passage rates by ethnicity. Your application will not be
deemed timely filed should you fail to submit this document
by the applicable filing deadline.
Of Verification. Print a hard copy of this form
as a single page and complete the information in pen. This
is your sworn verification that all statements and representations
in your application are true and correct. Signature and
date of notary execution must be no more than 30 days prior
to the date of filing. Your application will not be deemed
timely filed should you fail to submit this document by
the applicable filing deadline.
And Release. Print a hard copy of this form as
a single page and complete the information in pen. This
document authorizes the Board to conduct a background investigation
relative to your character and fitness for admission to
practice law. Signature and date of notary execution must
be no more than 30 days prior to the date of filing. Your
application will not be deemed timely filed should you
fail to submit this document by the applicable filing deadline.
